We are seeking a compassionate and team-oriented professional to work within a collaborative program designed to support older adults in maintaining independence.
In this position you will partner with connect with and arrange services to support older adults to remain living independently. You will build a consumer caseload and volunteer base by both engaging and partnering with others both within and outside of Family Service Rochester to build and maintain a relationship of trust. You will, with the input of individuals served and others jointly prepare a senior independence plan that outlines individual needs, wants and goals as well as next steps to extend living at home. The primary goal of our Aging Services Coordinator is to facilitate independent living; coordinate activities and community networks that help older adults and their care partners.

You will do this by:
- Completing consumer assessments, creating and monitoring independence plan implementation within the context of family, medical and the larger community in order to enhance the well-being and independent living in one’s own home.
- Consulting with care providers to help determine needs and conducting outreach and promoting the utilization of the Neighbors Helping Neighbors program in order to generate ongoing referrals of older adults who would benefit from NHN’s community-based services.
- Other important tasks within this position are: completion of required paperwork within established timeframes. Maintain data privacy in accordance with HIPAA and other appropriate practices, gain and maintain knowledge of community resources for partnership and referral.
- Working a schedule that is responsive to organizational and consumer needs, typically within normal business hours. You may be needed to participate in our rotating on-call weekend responsibilities for our Meals on Wheels programming.
